Our Board hasn't the backbone to accept that it's a living document and can/should be improved upon with time. Take cost estimates for example...Our Board considered outlawing them recently but didn't pass it, yet E&O is required in our state and every E&O (for HI's) that I've seen voids coverage as soon as a cost estimate is rendered in the report.
